I noticed that some of the Orange County engines are marked "Paramedic" and some are marked "Paramedic Assessment".  Is there an actual operational difference between the two or is it just cosmetic?

Paramedic assessment rigs have one paramedic, regular paramedic rigs have two. Depending on how the call is triaged you could get a paramedic assessment unit in first followed by a "paramedic unit" plus a a BLS ambulance.
