Several companies still make them Rosenbauer,Smeal and I think Pierce is offering them again. Not sure how cost effective it is over just buying a 4dr cab but I know at least you have the room to move around in the back of them so I am not sure why you dont see more of them seem pretty function on a commercial cab

One thing I can maybe see is cross lays are under the seats. I know if you got the 4dr cab the cross lays would be under the pump controls.  You save some wheel base there,not much but some.   I know the HME we got the cross lays wer under the controls and it added another Ft-1/2 to the truck.
