Looks to be former Miami, Florida Quint 7, one of three Young Crusader II LTI 75 foot quints built by Young 1991 Duplex D-350 mid-engine. The original bus style doors were replaced with a more conventional hinged door.

Looks like an identical spec to Columbus Twp Indiana s old quint which was originally Miami - Dade.
