Bethpage Maxim tiller..or simply ladder 3 as they are known on nassau fire radio.

 

This unit was eventually purchased by Anne Arundel County, MD (late 80's /early 90's I believe) and used as a reserve aerial. The attached photo (credit to the unknown photographer) shows the truck parked along the driveway at the County repair shop.
