the answer to question#2 it originally was in service with the greenwood hose co moosic,pa delivered december 1988

3) There were two built with Hahn bodies - Centre Square, PA and Fairlawn, NJ -  and one built with a Superior (Montana) body that went to Bostonia, CA.

Above and beyond the 45 confirmed 84/92's you mentioned, there are still a handful of Hahn rigs that could be 84/92's that I have never seen any photos of.
