Crew cab and pump panel configuration look like General Safety.  There were alot of these around the upper midwest

This Looks like the one that was listed on Brindle Mountain or Firetec For a while.  I can think of 3 Departments off the top of my head alone that had an engine like this in Minnesota. Hugo, Mendota Heights, and Eagan. I believe the one that was listed for sale was Hugo though.
